Non-food contact surfaces of equipment are unclean. OBSERVED THE RACKS IN THE REACH IN COOLER WHERE THE ORANGE JUICE IS HELD (ALL SHELVES IN THAT SECTION) ARE SOILED AND IN NEED OF CLEANING.

Physical facilities not maintained in good repair. OBSERVED THE CEILING TILE OVER THE APPLESAUCE AISLE WAS WATER DAMAGED AND IN NEED OF REPLACEMENT. PLEASE REPLACE ALL DAMAGED AND/OR MISSING CEILING TILES.
