Critical Repeat PIC unable to demonstrate knowledge by having no critical violations. OBSERVED CRITICAL VIOLATIONS IN THE FACILITY AT THE TIME OF THE INSPECTION. THE PIC SHOULD MAINTAIN MANAGERIAL CONTROL OVER THE FACILITY AT ALL TIMES IN ORDER TO PREVENT CRITICAL VIOLATIONS.

Critical Handwashing sink is being used for purposes other than handwashing. OBSERVED COFFEE BEING DUMPED AT THE PRIMARY HANDWASHING STATION. IN ORDER TO ALLOW FOR HANDWASHING ACCESSIBILITY, THE HANDWASHING SINK SHOULD ONLY BE USED FOR HANDWASHING. PLEASE DESIGNATE ONE SINK FOR COFFEE MAKING AND THE OTHER SINK FOR HANDWASHING.

Critical Repeat Equipment food-contact surfaces or utensils are unclean. OBSERVED THE CAPPUCHINO HOPPERS ARE SOILED. OBSERVED THE ICE CHUTE ON THE POP MACHINE IS SOILED WITH MILDEW. OBSERVED ALL THE COFFEE CANNISTER NOZZLES ARE VERY SOILED. IN ORDER TO PREVENT CONTAMINATION, ALL FOOD CONTACT SURFACES ARE TO BE KEPT CLEAN TO SIGHT AND TOUCH. PIC INSTRUCTED TO CLEAN ABOVE FOOD CONTACT SURFACES.

Critical Corrected During Inspection Ready-to-eat, TCS food not properly discarded when required. OBSERVED THE HOTDOGS WERE DATED WITH A DISCARD DATE OF 12/6. IN ORDER TO PREVENT THE GROWTH OF PATHOGENS, ALL TCS FOOD MUST BE DISCARDED WITHIN 7 DAYS OF OPENING THE PACKAGE OR PREPARATION. HOTDOGS WERE DISCARDED.

Repeat No towels or drying device at the handwashing sink(s). OBSERVED NO PAPER TOWELS AT EITHER HANDWASHING SINK.

Hot and cold holding equipment thermometer was missing, located incorrectly, or not easily readable. OBSERVED THE GAUGE ON THE WALK IN FREEZER WAS BROKEN.

Improper storage of food items. OBSERVED FOOD ON THE GROUND IN THE WALK IN FREEZER.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Repeat Non-food contact surfaces of equipment are unclean. OBSERVED DRAWERS UNDER SLUSHY AND CAPPUCHINO MACHINES ARE SOILED. OBSERVED THE HANDWASHING SINK BY THE 3 COMPARTMENT SINK IS SOILED.

Repeat Physical facilities not maintained in good repair. OBSERVED THE LIGHT SHIELD BY THE 3 COMPARTMENT SINK IS DAMAGED AND IN NEED OF REPLACEMENT. OBSERVED A MISSING HVAC VENT BY THE WALK IN FREEZER.
