Repeat Physical facilities not maintained in good repair. OBSERVED THE ROOF WAS DAMAGED IN THE STORAGE ROOM TO THE BACK OF THE FACILITY. THE FACILITY HAS BEEN IN CONTACT WITH THE LANDLORD REPEATEDLY THROUGHOUT THE YEAR TO RESOLVE THIS ISSUE AND THE LANDLORD RECENTLY HIRED A COMPANY TO REPAIR THE CEILING AND ROOF.

OBSERVED FOOD EMPLOYEES WEARING GLOVES AND CHANGING GLOVES APPROPRIATELY WHILE PREPARING FOOD FOR SERVICE. IN ORDER TO PREVENT CONTAMINATION, FOOD EMPLOYEES SHOULD WEAR GLOVES WHILE PREPARING READY TO EAT FOOD ITEMS.

VI - P - TCS Food: Observed hot foods being held at 135°F or above; cold foods being held at 41°F or below. OBSERVED ALL TCS FOOD ITEMS BEING HELD AT THE PROPER TEMPERATURES. OBSERVED SOUP, CHICKEN, AND SLICED TOMATOES HOLDING AT 41 DEGREES F. IN ORDER TO PREVENT THE GROWTH OF PATHOGENS, ALL TCS FOOD SHOULD BE KEPT OUT OF THE TEMPERATURE DANGER ZONE WHICH EXISTS ABOVE 41 DEGREES F. AND BELOW 135 DEGREES F.

X - Chemical: Observed toxic materials improperly identified, stored and used. OBSERVED TOXIC CHEMICALS SUCH AS FRYER CLEANER AND DRAIN CLEANER ON THE SAME SHELF AS THE SINGLE USE CUPS. IN ORDER TO PREVENT CONTAMINATION, TOXIC CHEMICALS SHOULD BE KEPT IN A DESIGNATED AREA AWAY FROM FOOD, DISHWARE, AND UTENSILS. PIC HAD AN EMPLOYEE MOVE ALL THE CHEMICALS AT THE TIME OF THE INSPECTION TO A DESIGNATED AREA.
