Critical Observed the air gap between a plumbing fixture and a piece of equipment that was not sufficient to prevent back siphonage of contaminated water. OBSERVED INSUFFICIENT AIR GAP UNDER PREP SINK TO PREVENT BACKFLOW OF CONTAMINATED WATER.
To prevent contamination, an air gap between the water supply inlet and the flood level rim of the plumbing fixture, equipment, or nonfood equipment shall be at least twice the diameter of the water supply inlet and may not be less than one inch (25 millimeters). TO CORRECT CUT PIPE SO THAT A SUFFICIENT AIR GAP IS PROVIDED UNDER PREP SINK TO PREVENT BACKFLOW OF CONTAMINATED WATER. Correct By: 26-Dec-2017

Observed no handwashing sign(s) posted at handwashing sink(s) used by employees. OBSERVED NO HANDWASHING SIGNS AT HANDWASH SINK INSIDE SERVING AREA OR MENS RESTROOM.
A sign or poster that notifies food employees to wash their hands shall be provided at all handwashing sinks used by food employees. TO CORRECT PROVIDE HANDWASH SIGNAGE AT SERVING AREA HAND WASHING SINK AND MENS RESTROOM.
