Critical The person in charge did not ensure that employees are properly sanitizing equipment or utensils.( MANAGER INSUFFICIENT IN KNOWLEDGE IN REGARDS TO CLEANING AND SANITIZING THE ICE MACHINE USED IN THE OPERATION AT TIME OF INSPECTION)

Facility did not have one employee with supervisory and management responsibility and the authority to direct and control food preparation and service with level two certification in food protection.( PROVIDE LEVEL TWO INFORMATION NOT AVAILABLE AT TIME OF INSPECTION. OPERATION SHALL SUBMIT RECIPROCITY APPLICATION TO ODH FOR THE CERTIFICATION IN FOOD PROTECTION)

Working containers holding food or food ingredients that are removed from their original packages shall be identified with the common name of the food, except containers holding food that are readily and unmistakably recognized.
3717-1-04.4(B) / Cutting surfaces.
The surfaces of cutting blocks or boards were severely scratched or scored and could not be effectively cleaned and sanitized.
Surfaces such as cutting blocks and boards that are subject to scratching and scoring shall be resurfaced if they can no longer be effectively cleaned and sanitized, or discarded if they are not capable of being resurfaced.
3717-1-04.8(G) / Kitchenware and tableware - handling.
Single-service or single-use articles, kitchenware, or tableware were not handled, displayed, or dispensed properly.( LOCATION NEAR THE BEVERAGE AREA OF THE OPERATION)
Single-service articles or single-use articles and cleaned and sanitized utensils shall be handled, displayed, and dispensed so that contamination of food-contact-and lip-contact surfaces is prevented as described in this rule.
3717-1-05.1(S)(2) / Plumbing system - maintained in good repair.
The plumbing system was not properly maintained.( HAND SINK IN THE GRILL PREP. SINK AREA NOT WORKING AT TIME OF INSPECTION IN NEED OF REPAIRS)
A plumbing system shall be properly maintained.( PROVIDE THE NECESSARY REPAIRS TO HAND SINK TO ENSURE SUFFICIENT NUMBER OF WORKING HAND SINKS IN THE FOOD PREP. AREA MAINTAIN AT ALL TIMES)
