Repeat Facility does not have an employee with manager certification in food protection. OBSERVED NO PERSON WITH AN OHIO MANAGERS CERTIFICATION IN FOOD PROTECTION.

3717-1-03.4(B) / Time/temperature controlled for safety food - slacking.
Food improperly slacked. OBSERVED LAMB DEFROSTING IN A TUB OF WATER. FOOD EMPLOYEE INSTRUCTED TO EITHER DEFROST THE LAMB UNDER COLD RUNNING WATER OR USE THE WALK IN COOLER TO DEFROST THE LAMB.

3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Repeat Equipment components are not intact, tight or properly adjusted. OBSERVED THE GASKETS ON THE TWO DOOR REACH IN COOLER ARE DAMAGED. OBSERVED THE RIGHT SIDE DOOR OF THE REACH IN FREEZER HAS A DAMAGED GASKET.

Non-durable equipment observed. OBSERVED THE BLENDER CANNISTER WAS CRACKED. PLEASE DO NOT USE THIS BLENDER. A REPLACEMENT CANNISTER WILL HAVE TO BE FOUND OR THE BLENDER WILL HAVE TO BE REPLACED.

I - P - Employee Health: The operation had an employee health policy on file. OBSERVED AN EMPLOYEE HEALTH POLICY FOR ALL EMPLOYEES. IN ORDER TO PREVENT THE SPREAD OF FOODBORNE ILLNESS, ALL EMPLOYEES MUST READ AND SIGN THE EMPLOYEE HEALTH POLICY.

III - P - Preventing Contamination by Hands: Hand washing facilities are properly supplied. OBSERVED THE HANDWASHING SINK IS PROPERLY SUPPLIED WITH WARM WATER, SOAP, PAPER TOWELS, AND A HANDWASHING SIGN. IN ORDER TO PROMOTE GOOD HANDWASHING, THE HANDWASHING SINK MUST ALWAYS BE FULLY STOCKED.

X - P - Chemical: Toxic materials are properly identified and stored. OBSERVED TOXIC CHEMICALS ARE STORED IN DESIGNATED AREA. IN ORDER TO PREVENT CONTAMINATION, ALL TOXIC CHEMICALS SHOULD BE STORED IN A DESIGNATED AREA AWAY FROM FOOD, DISHWARE, AND UTENSILS.
