3717-1-02.4(A)(2) / Person in charge: assignment of responsibility.
Repeat Facility did not have one employee with supervisory and management responsibility and the authority to direct and control food preparation and service with level two certification in food protection. OBSERVED THAT THE LEVEL TWO CERTIFICATION IN FOOD PROTECTION HAS NOT BEEN OBTAINED.
At least one employee that has supervisory and management responsibility and the authority to direct and control food preparation and service shall obtain the level two certification in food protection according to rule 3701-21-25 of the Administrative Code. TAKE AN OHIO DEPARTMENT OF HEALTH (ODH) FIFTEEN HOUR APPROVED FOOD SAFETY TRAINING COURSE, OBTAIN THE COURSE CERTIFICATE UPON COMPLETION, THEN SUBMIT A COPY OF THE COURSE CERTIFICATE, A COPY OF THE COURSE CURRICULUM, AND THE COMPLETED RECIPROCITY APPLICATION TO ODH TO SEEK POSSIBLE APPROVAL FOR THE LEVEL TWO CERTIFICATION IN FOOD PROTECTION. UPON APPROVAL, THE CERTIFICATION WOULD BE GENERATED.
3717-1-04.2(G)(1) / Food temperature measuring devices - availability
A food thermometer was not readily accessible. OBSERVED THAT A PROBE FOOD THERMOMETER WAS NOT AVAILABLE.
To prevent foodborne illness, food temperature measuring devices shall be provided and readily accessible for use. PROVIDE A PROBE FOOD THERMOMETER TO VERIFY INTERNAL FOOD TEMPERATURES.
3717-1-06.1(A) / Floors, walls, and ceilings.
Floors, walls, and/or ceilings were not smooth and easily cleanable. OBSERVED WOOD PANELING IN VARIOUS PLACES ON THE CEILING.
Floors, floor coverings, walls, wall coverings, and ceilings shall be designed, constructed, and installed so they are smooth and easily cleanable. MAKE NECESSARY ADJUSTMENTS/POSSIBLE REPAIRS TO PROVIDE A SMOOTH AND EASILY CLEANABLE CEILING SURFACE.
3717-1-06.2(E) / Handwashing signage.
Corrected During Inspection Observed no handwashing sign(s) posted at handwashing sink(s) used by employees. OBSERVED NO HANDWSHING SIGN AT THE KITCHEN HANDSINK OR THE UNISEX RESTROOM HANDSINK.
A sign or poster that notifies food employees to wash their hands shall be provided at all handwashing sinks used by food employees. TWO HANDWASHING SIGNS WERE PROVIDED AND POSTED DURING THE TIME OF INSPECTION. Correct By: 21-Aug-2018

Observed unnecessary or nonfunctional items and /or litter on the premises. OBSERVED SHOPPING CARTS AND BUILDING MATERIALS BEHIND AND NEAR THE DUMPSTER.
The premises shall be free of Items that are unnecessary to the operation or maintenance of the FSO or RFE such as equipment that is nonfunctional or no longer used and litter. REMOVE ALL ITEMS FROM THE DUMPSTER TO PREVENT POSSIBLE PEST/RODENT INFESTATION AND POSSIBLE HARBORAGE.
3717-1-06.4(B) / Cleaning - frequency and restrictions.
Observed a build-up of dirt and debris. OBSERVED A BUILD-UP OF FOOD DEBRIS AND DIRT ON THE FLOORS, UNDERNEATH THE COOKING EQUIPMENT, BEHIND THE COOKING EQUIPMENT AND ON THE COUNTERTOPS AROUND THE MEAT SLICER AND THE HOT HOLDING UNIT.
The physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed. THOROUGHLY CLEAN THE ENTIRE KITCHEN, INCLUDING THE FLOORS, COOKING EQUIPMENT AND UTENSILS TO PREVENT POSSIBLE PEST/RODENT INFESTATION AND POSSIBLE HARBORAGE.
