Repeat Facility does not have an employee with manager certification in food protection. OBSERVED NO PERSON WITH A OHIO MANAGERS CERTIFICATION IN FOOD PROTECTION. PIC HAS A SERVSAFE AND WILL TURN IT IN WITH THE APPLICATION FOR RECIPROCITY TO RECEIVE THE LEVEL TWO CERTIFICATE.

No sanitizer test kit available. OBSERVED NO SANITIZER TEST KIT FOR THE CHLORINE.

3717-1-06.4(A) / Repairing.
Physical facilities not maintained in good repair. OBSERVED THE KITCHEN FLOOR WAS MISSING A PART OF THE LINOLEUM AND WAS WRINKLED UNDER THE 3 COMPARTMENT SINK. PLEASE REPAIR OR REPLACE THE FLOORING.
