3717-1-04.5(D) Repeat Non-food contact surface(s) not cleaned at the required frequency. Observed build up of food and grease on equipment and floors throughout the FSO. Ensure that nonfood-contact surfaces of equipment are cleaned at a frequency that precludes a build up of soil residues.

3717-1-04.5(A)(1)/Cleanliness of equipment food-contact surfaces and utensils. Equipment food-contact surfaces or utensils are unclean. Observed brown, slimy residue in the ice machine. Ensure that food contact surfaces of equipment are clean to sight and touch.
3717-1-06.4(K)/Controlling pests. Presence of live insects, rodents, and other pests. Observed two juvenile cockroaches. Ensure that licensed pest control is provided, harborage conditions are eliminated, and food sources (such as grease and food debris caked on equipment, floors, and walls) are cleaned up.
3717-1-04.1(C)/Food-contact surfaces - cleanability Food contact surfaces not easily cleanable. Observed cutting boards with deep cuts and black residue. Ensure that food contact surfaces are free of breaks, open seams, cracks, chips, inclusions, pits, and similar imperfections.
3717-1-04.4(D)/Warewashing equipment - cleaning frequency. Warewashing equipment and/or components not cleaned at required frequency. Observed three compartment sink was being used as storage for a number of items. Ensure that three compartment sink is maintained clean. Remove any unnecessary items from the FSO.
3717-1-05.4(K)/Storing refuse, recyclables, and returnables. Refuse or recyclables stored so they are accessible to insects and/or rodents. Observed refuse and clothes piled up beneath ventilation duct. Ensure that refuse is stored in receptacles or waste handing units so that they are inaccessible to insects and rodents.
3717-1-06.4(B)/Cleaning - frequency and restrictions. Facility not maintained clean. Observed cluttered shelves, dirty floors and walls, and unnecessary items throughout the FSO. Ensure that facility is maintained clean.

Comments
Upon inspection, facility was much cleaner and new additions were made to the master cleaning schedule to help maintain cleanliness. Pest control is continuing to be provided on a monthly basis, or at the frequency recommended by the licensed pest control professional.
