3717-1-06.4(B) Facility not maintained clean. Observed bar area and dining area with extensive food debris and trash on the floors under the booths and under the bar equipment. Please maintain this area clean to the sight and touch. Please increase the frequency the cleaning company is coming to help maintain this area and avoid an environment conducive to pests.

Complaint was received stating roaches were seen in the bar area and the warewashing area. Investigation did not present any evidence of roaches, but facility has recently switched pest control company and getting better results. Please continue with new pest control company as treatment appears to be working. Old mouse feces were observed in various areas. Please continue with current pest control and deep clean all areas of the facility to rid of old evidence and prevent harborage. See comment #62.
