Critical Observed food that was not properly protected from contamination by separation, packaging, and segregation. RAW STEAKS STORED OVER PITA BREAD & RAW CHICKEN STORED OVER SMOKED HAMS IN WALK-IN FREEZER. FOOD ITEMS NOT COVERED IN STORAGE IN REACHIN COOLER & FREEZERS.
To prevent contamination, food shall be protected from cross contamination by separating raw animal foods during storage, preparation, holding, and display from cooked ready-to-eat food as described in this rule.STORE RAW FOOD ITEMS BELOW READY TO EAT ITEMS AND PER THE COOKING TEMP HIERARCHY. During preparation, unpackaged food shall be protected from environmental sources of contamination. PIC THREW CONTAMINATED PASTA AWAY. HE REMOVED POT TO ALLOW IT TO AIR DRY.

Observed light bulbs or heat lamp that were not properly shielded or coated where required. NO SHIELDS ON LIGHTS IN STORAGE BUILDING.
Light bulbs shall be shielded, coated, or otherwise shatter-resistant in areas where there is exposed food, clean equipment, utensils, or linens, or unwrapped single-service or single-use articles and does not apply in areas used only for storing food in unopened packages. An infrared or other heat lamp shall be protected against breakage by a shield surrounding and extending beyond the bulb so that only the face of the bulb is exposed.

Observed employees drinking where food, equipment, or utensils may be contaminated. EMPLOYEE DRINKS FOUND IN COOLERS AND ON PREP TABLES. PURSE FOUND ON TOP OF FREEZER.
Areas designated for employees to eat, drink, and use tobacco shall be located so that food, equipment, linens, and single-service and single-use articles are protected from contamination. PROVIDE AN AREA FOR ALL EMPLOYEE PERSONAL ITEMS.

Observed a build-up of dirt and debris. WALL BEHIND DISHMACHINE, WALL NEXT TO OFFICE, SIDE OF DUMP SINK & AREA AROUND OUTLETS & POWER CORDS ARE DIRTY & NEED CLEANED.
The physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed.

Observed unnecessary or nonfunctional items and /or litter on the premises. BOTH STORAGE BUILDING AND OFFICE HAVE FOOD, PAPER PRODUCTS, OLD EQUIPMENT, CLEANERS, TOOLS, AND OFFICE/BUSINESS ITEMS STORED IN THEM. NEITHER AREA IS ORGANIZED TO INDICATE WHERE FOOD AND PAPER PRODUCTS NEED STORED TO PREVENT CONTAMINATION.
The premises shall be free of Items that are unnecessary to the operation or maintenance of the FSO or RFE such as equipment that is nonfunctional or no longer used and litter. ORGANIZE STORAGE BUILDING AND OFFICE AREA. STORE ALL FOODS AND PAPER PRODUCTS (NAPKINS, CUPS, LIDS) SEPARATE AND ABOVE ALL OTHER ITEMS. REMOVE ALL ITEMS UNNECESSARY TO THE OPERATION.
