Observed no service sink or curbed cleaning facility as required. HOLE IN SIDE OF FIBERGLASS TUB IN STORAGE BUILDING. REPLACE TUB WITH A UTILITY TUB THAT CAN BE USED AS YOUR MOP SINK.
At least one service sink or one curbed cleaning facility equipped with a floor drain shall be provided and conveniently located as specified in this rule.

Critical Handwashing sink is being used for purposes other than handwashing. OBSERVED MUSHROOMS BEING WASHED IN A COLANDER IN THE HANDSINK
To prevent contamination, a handwashing sink may not be used for purposes other than handwashing.

Critical Observed a direct connection between the sewage system and a drain originating from equipment in which food, portable equipment, or utensils are placed. NO AIR GAP AT ICE MACHINE DRAIN IN STORAGE BUILDING.
To prevent health hazards, a direct connection may not exist between the sewage system and a drain originating from equipment in which food, portable equipment, or utensils are placed unless allowed by the Ohio building code for: a warewashing machine or a warewashing and culinary sink.

Observed light bulbs or heat lamp that were not properly shielded or coated where required. LIGHTS IN STORAGE BUILDING & LIGHT IN WAITRESS AREA NOT PROTECTED WITH SHIELDS.
Light bulbs shall be shielded, coated, or otherwise shatter-resistant in areas where there is exposed food, clean equipment, utensils, or linens, or unwrapped single-service or single-use articles and does not apply in areas used only for storing food in unopened packages. An infrared or other heat lamp shall be protected against breakage by a shield surrounding and extending beyond the bulb so that only the face of the bulb is exposed.

Observed a build-up of dirt and debris. FLOOR IN STORAGE BUILDING HAS A BUILD UP ESPECIALLY IN HIGH TRAFFIC AREAS.
The physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed.
