Food employee(s) not wearing a hair restraint. Observed many workers on the food line with beards greater than an inch in length who were not wearing beard restraints. Please provide and require beard restraints for workers.

Studs, joists, and/or rafters exposed in areas subject to moisture. Observed ceiling tiles above prep line were slid open exposing roof and rafter areas. Please ensure the ceiling tiles are kept closed at all times above food prep areas. PIC stated he had been working there earlier in the day to determine a leak.

Facility not maintained clean. Observed handsink near oven not clean. Please ensure that handsinks are kept clean and sanitized. Also observed freezer floors with spilled food and trash. Please keep freezer floors clean.
