3717-1-04.1(C) / Food-contact surfaces - cleanability
Critical Food contact surfaces were not easily cleanable, or not designed/constructed properly. OBSERVED THE FOLLOWING: 1. THE SCISSORS TO CUT THE DISPENSING TUBES IS NOT CLEANABLE. 2 THE COVERS ON THE ICE CREAM MACHINE ARE CHIPPED. REPAIR/REPLACE
To prevent the growth of pathogens, Multiuse food-contact surfaces shall be: (1) Smooth; (2) Free of breaks, open seams, cracks, chips, inclusions, pits, and similar imperfections; (3) Free of sharp internal angles, corners, and crevices; (4) Finished to have smooth welds and joints; and (5) Except for cooking oil storage tanks, distribution lines for cooking oils, or beverage syrup lines or tubes; accessible for cleaning and inspection by one of the following methods: (a) Without being disassembled; (b) By disassembling without the use of tools; or (c) by easy disassembling with the use of handheld tools commonly available to maintenance and cleaning personnel such as screwdrivers, pliers, open-end wrenches, and Allen wrenches.

Corrected During Inspection Observed a FSO that did not display their license at the facility; Observed a mobile FSO that did not display all required information on the exterior of the unit. FSO DID NOT HAVE THEIR MOST CURRENT LICENSE. THE CORPORATE OFFICE WAS CALLED AND A COPY WAS EMAILED AS A TEMPORARY FIX
A licensee shall display the license for that food service operation at all times at the licensed location; Each operator of a mobile FSO shall conspicuously display the name of the operation, city of origin, and area code and telephone number on the exterior of the mobile unit with the individual lettering measuring at least three inches high by one inch wide.

Observed no covered receptacle in women's restroom. OBSERVED NO COVERED RECEPTABLE IN THE WOMAN'S RESTROOM. OBTAIN
A toilet room used by females shall be provided with a covered receptacle for sanitary napkins.
3717-1-06.4(H) / Cleaning and maintenance of plumbing fixtures.
Corrected During Inspection Observed plumbing fixtures that were not being kept clean and maintained. NOTED GRIME/BLACK SUBSTANCE IN/ON THE HANDWASHING SINKS. CORRECTED
Plumbing fixtures such as handwashing sinks, toilets, and urinals shall be cleaned as often as necessary to keep them clean and maintained.
3717-1-07.1(P) / Personal care items - storage.
Corrected During Inspection Observed personal care items that were not stored in a designated employee storage area. OBSERVED A PERSONAL CELL PHONE ON THE SQUARE TABLE IN DRIVE THRU. CORRECTED. DO NOT STORE OR USE PERSONAL CELL PHONES IN THE KITCHEN. THEY ARE UNSANITARY.
Employees shall store their personal care items in facilities as specified under (K)(2) of rule 3717-1-6.2 of this code.
