Repeat The physical facilities are not being maintained in good repair. THE FLOOR TILE GROUT LEVEL IS VERY LOW BY THE FRYERS AND BY THE DISHWASHING MACHINE/3 COMPARMTENT SINK AREA. ALL LOW FLOOR TILE GROUT LEVELS NEED TO BE REPAIRED TO PREVENT WATER, DIRT, AND DEBRIS ACCUMULATION.
The physical facilities shall be maintained in good repair.

PERFORMED A STANDARD AND COMPLAINT INSPECTION. IN REGARDS TO THE COMPLAINT, THERE WAS A URINAL IN THE MEN'S REST-ROOM THAT WAS OUT OF ORDER, AND THE GM HAD ALREADY PLACED A SERVICE CALL ON IT. OBSERVED SOAP AND HAND DRYING UNITS TO BE AVAILABLE/WORKING IN THE MEN'S AND WOMEN'S REST-ROOM. ALSO, OBSERVED HOT WATER TO BE AVAILABLE AFTER LETTING THE WATER RUN FOR ABOUT 60 SECONDS. COMPLAINT CLOSED.
