Critical Corrected During Inspection Employees are not informed in a verifiable manner of their responsibility to report information about their health. Management was able to print out an acceptable sick policy and had employees read and sign the policy. PIC stated they will get all employees to review and sign.

3717-1-03 / Food - safe, unadulterated, and honestly presented
Food was unsafe, adulterated, or not honestly presented. Observed avocado spread past the use by date displayed at the front refrigeration unit. PIC removed the product and voluntarily discarded.

3717-1-02.2(G) / Jewelry - prohibition.
Food employees wearing jewelry on arms or hands (watches and bracelets) during food preparation.

3717-1-04.8(E)(1) / Single-service and single-use articles - storage
Improper storage of single-service and single-use articles. Observed multiple boxes stored on the floor throughout the back room.

3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Equipment components are not intact, tight or properly adjusted. Observed wiping clothes wrapped around the hose connection for the rinse feature equipment in both sinks at the front counter area. Please provide documentation that the hose connection is a backflow prevention device.

3717-1-04.3(B) / Fixed equipment installation - spacing or sealing.
Fixed equipment not properly sealed or spaced for cleaning. Observed damaged caulking around the two hand sinks at the front counter area and around the sink with the rinse feature.
