[?] A summary of the violations found during the inspection are listed below.

3717-1-05.3(C) / Sewage and other liquid waste: Backflow prevention.
Critical Observed a direct connection between the sewage system and a drain originating from equipment (sink for washing/preparing foods) in which food, portable equipment, or utensils are placed at the time of inspection. Facility was not provided with a properly installed sink for preparing ready to eat foods (vegetables and fruits).
To prevent contamination to foods, a direct connection may not exist between the sewage system and a drain originating from equipment in which food, portable equipment, or utensils are placed unless allowed by the Ohio building code. If foods are to be washed and prepared at this location, then a designated food preparation sink with a indirectly connected drain shall be provided. Until this sink has been provided, all foods that are required to be washed shall be washed at the Stanton Middle School wrapped and then delivered ready to eat.
3717-1-06.4(K) / Controlling pests.
Critical Observed the presence of rodents or pests at the time of inspection. Droppings were found throughout the cupboards under the serving window. PIC stated evidence of mouse was recently found and maintenance was notified.
To prevent illness, the presence of insects, rodents, and other pests shall be controlled by: Routinely inspecting incoming shipments of food and supplies; routinely inspecting the premises for evidence of pests; Using methods, if pests are found, such as trapping devices or pesticides used according section (C), (K) and (L) of rule 3717-1-07.1 of this code; and eliminating harborage conditions. Recommend contacting a licensed pest control operator to ensure the FSO is pest free.

3717-1-02.4(A)(2) / Person in charge: assignment of responsibility.
Observed facility did not have one employee with supervisory and management responsibility and the authority to direct and control food preparation and service with level two certification in food protection at the time of inspection.
To meet the requirements of this rule, at least one employee that has supervisory and management responsibility and the authority to direct and control food preparation and service shall obtain the level two certification in food protection according to rule 3701-21-25 of the Administrative Code. Provide a level 2 ODH certification in food protection for a proper employee of the FSO.
3717-1-02.4(C)(16) / Person in charge: duties - ensure the facility has written procedures for vomiting and diarrheal events.
Observed the person-in-charge did not ensure that the facility has written procedures for employees to follow when responding to vomiting or diarrheal events. PIC stated custodial service would handle all events.
To meet the requirements of this rule, person in charge shall ensure that the FSO have written procedures for employees to follow when responding to vomiting or diarrheal events that involved discharge onto surfaces. Written procedure shall be developed and implemented in the FSO.
3717-1-05.1(S)(2) / Plumbing system - maintained in good repair.
Observed the plumbing system for the warewashing sink (3-compartment) not properly maintained at the time of inspection. When all 3 sinks drain water overflows the grease trap and leaks on the floor.
To prevent health hazards, a plumbing system shall be properly maintained. Repair plumbing system to be in good working order.

Observed PIC washing hands properly and when required. PIC demonstrated good knowledge in food safety.
Equipment or items that are used by employees only shall be identified and kept separate.
A slight odor was found from the grease trap, ensure grease trap is maintained.
The violations documented on this report will be checked during next standard inspection.
