| This inspection was completed per the the request of the owner to approve MFSO trailer to be licensed. The MFSO is approved with the understanding the following will be addressed: 1) The potable water tank shall be serviced before operating. 2) Recommend contacting the Kent Fire Prevention to have the trailer inspected. Observed a gas smell at the time of inspection. Be prepared to have the gas lines pressure tested. 3) Ensure there is a diarrheal/vomit clean up kit with proper procedures. 4) Provide verification that all staff present have been trained on the symptoms and illnesses to report to PIC. 5) Ensure at least one person with Person In Charge certification is present during all times of operation. Commissary location is currently a the Erie Street Kitchen FSO. Owner is in the process of obtaining processor approval from ODA for a warehouse on Temple Ave. Please provide the ODA registration numbers for both locations. Okay to operate. A signed application and license fee for the amount of $310 (Check# 1474) was received at the time of inspection. License will be mailed once processed. |
