Comments
This inspection was completed per the the request of the owner to approve an additional trailer to the MFSO and to operate at the Kent Skate Ice Rink. Owner has provided documentation that the trailer and cart will be used together as one licensed operation. They would like the option to use one or the other or both depending on the event. Documentation provided indicates that if the cart and trailer were to be used at the same time at different locations, a second mobile or temporary FSO license will be obtained before operating. Owner is aware that 2 mobile licenses may be required next year.
The Trailer/MFSO is approved with the understanding the following will be addressed:

1) The potable water tank shall be serviced before operating. Refer to EPA guidance on the proper steps to take.
2) Owner stated a food grade hose with proper backflow prevention device (ASSE 1024 LF) is at the commissary location.
3) Recommend contacting the Kent Fire Prevention to have the trailer inspected.
4) A new layout has been provided and will be added to the license next licensing renewal.
5) Ensure there is a diarrheal/vomit clean up kit with proper procedures.
6) Provide verification that all staff present have been trained on the symptoms and illnesses to report to PIC.
7) Ensure at least one person with Person In Charge certification is present during all times of operation.

The situation with the Trailer and cart will be reviewed during the renewal pre-licensing inspection and all items are subject to change upon additional review.
