Observed bottom shelf of multiple shelving units in the basement installed on the floor and crates being used as shelving for pizza boxes.

3717-1-04.8(E)(3) / Single-service articles and single-use articles - storage.
Single-service and single-use articles not protected from contamination. Observed unwrapped boxes being stored on crates in the basement.

3717-1-04(I) / Nonfood-contact surfaces - materials.
Nonfood-contact surfaces constructed of unapproved materials. Observed wood material used throughout the facility.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Observed an accumulatrion of food residue and other debris on the following non-food contact surfaces of equipment: 1) shelving throughout the facility, specifically above the three compartment sink. 2) plumbing fixtures for three-compartment sink and hand sink. 3) dough mixer (PIC stated this equipment was just used).

Floors and floor and wall junctures are not maintained clean throughout the main room.

3717-1-06.1(A) / Floors, walls, and ceilings.
Floors, walls, and/or ceilings not smooth and easily cleanable. Observed damaged tile near the pizza oven and damaged walls throughout main room.

3717-1-06.4(N) / Maintaining premises - unnecessary items and litter.
Unnecessary or nonfunctional items and /or litter on premises. Observed a pile of items in the basement that is no longer used (including 2 chest freezers and paint).

Comments
Observed properly installed food preparation sink next to the pizza oven. Discussed cleaning and sanitizing before and after use.
Ingredient listing shall be available for products (frozen) removed from packaging to help create more storage space.
Per discussion with PIC, a separate container of tomatoes will be used for salads to prevent contamination.
Mops shall be properly air dried, recommend providing a workable mop hanger.

This 30 day/standard was conducted during the COVID-19 pandemic and was a focus during the inspection.
The violations documented shall be corrected and will be checked during next standard inspection.
