[?] A summary of the violations found during the inspection are listed below.

3717-1-06.4(A) / Repairing.
Observed two large chunks of ice in the walk-in freezer and trays ontop of product to catch condensation. Discontinue storing foods under the discharge pipe if they are leaking. Ice accumulation shall not occur.

Observed multiple holes in the FRP wall material in the FSO (specifically at three-compartment sink). Observed damaged corners around the column in the middle of the kitchen. Observed unknown substance above the hand sink and three-compartment sink where the wall meets the ceiling.
