3717-1-05.1(O)(1)
Corrected During Inspection Critical Handwashing sink not accessible. Observed chemicals and items being stored on the handwashing sink. PIC removed items and will talk with employees about proper handwashing and having the sink accessible at all times.

3717-1-04.5(A)(1)
Corrected During Inspection Critical Equipment food-contact surfaces or utensils are unclean. Observed container of dirty knives in the back prep area. PIC stated some of the knives are no longer used. Equipment and utensils that are no longer used shall be removed from the facility. PIC pulled the knive that is used from the container to be warewashed.

Label on a food packaged in-house did not contain all required information. Observed labels without address on multiple packaged products throughout the facility. Observed trout labeled as chicken. PIC removed products and will ensure proper labels are on the packages before being sold.

Repeat A food packaged in the facility not labeled. Observed packaged food in the front refrigeration unit and frozen packages at the display freezers withoout labels (labels were displayed on the glass). PIC removed these items from sale until labels can be placed on package.

Repeat Unpackaged foods improperly stored/displayed in direct contact with undrained ice. Observed raw shrimp and other fish in containers with ice at the end of the long produce display refrigeration unit.

Equipment not approved by a recognized testing agency. Observed non-commercial reciporicating saw with meat debris and PIC stated they use it to cut through bones when portioning/packaging meats. Discussed discontnuing using unapproved equipment and purchasing a commercial bone saw.

Non-food contact surfaces throughout the facility (shelving, equipment, prep sink area, three-compartment sink area not cleaned at the required frequency to prevent build up of dirt and debris

Repeat Unnecessary or nonfunctional items and /or litter on premises. Observed multiple uneccesary items such as building tools, car equipment, etc. that shall be seperated and stored away from the operation.

Observed floors thoughout the facility not maintained clean. Any damaged flooring shall be replaced to allow for easy cleaning. Boxes shall be off the floor to allow for easy cleaning.

Comments
The violations documented shall be corrected and will be checked during next standard inspection.
PIC stated the pork that is being cured in the racks outside is for employee meals and is not sold to customers. If product is to be sold to customers, the process may require a variance from Ohio department of agriculture.
Discontinue using non-commercial saws to portion meats. Provide information on equipment to the health department prior to purchasing to ensure the equipment is approved.
