Comments
This standard inspection was completed per the request of the operator approve the MFSO to be issued a license. MFSO was not in operation at the time of inspection. The MFSO has been approved with the understanding that the following shall be completed:

1) Provide a new sign in 3 by 1 inch font stating the City, State and phone number. Recommend changing the operator name from Kent State University Dining Services to the new licensee name.
2) Provide documentation regarding ODA processing or warehouse license requirements. If ODA claims nothing is required, provide documentation from ODA stating this.
3) Clean and reseal the gap to the left of the flat top grill.
4) Caulk all counter top and wall junctures. Recommend checking all junctures and seal ass needed to prevent food traps.
5) Improve the single-use item storage to be in compliance with the food code.
6) Provide a potable/drinking water standard grade hose or provide documentation that the current hose in office meets these requirements.
7) Recommend documented the service events of the potable water container.
8) Ensure at least one person with supervisory duties is PIC certified while the MFSO in operation.
9) Provide verification documentation that all staff present at time of operating are trained on the symptoms and illnesses to report.
10)Return the completed application with proper payment of $310 before operating.
11) Please review the KFD requirements and comply accordingly.
12) Provide procedures and kit for vomit events.
13) Ensure all cooler are properly holding at 41 degrees F or below. Recommend developing s TILT (time instead of temperature procedure) incase there is ever a cold or hot holding issue.

Note: If the menu has change, provide an updated menu to be included on the back of the MFSO license.
