| This inspection was completed per the the request of the owner to approve MFSO trailer to be licensed. The MFSO is approved with the understanding the following will be addressed: 1) Recommend contacting the Kent Fire Prevention to have the trailer inspected. 2) Wooden items are not permitted as counter tops. Any wood shall be replaced or covered by an approved material. 3) Ensure there is a diarrheal/vomit clean up kit with proper procedures. 4) Provide verification that all staff present have been trained on the symptoms and illnesses to report to PIC. 5) Ensure at least one person with Person In Charge certification is present during all times of operation. 6) Ensure a metal stem thermometer is available. Commissary location is currently a the Erie Street Kitchen FSO. Owner stated this location has been renewed as a processor with ODA and will forward the registration details when able. There is a ODA for a warehouse on 632 Temple Ave. ODA registration 4743673 #6700525. Please send copies of both documents for the Licensor file. Okay to operate. A signed application and license fee for the amount of $330 (Check# 1516) was received at the time of inspection. License will be mailed once processed. |
