[?] A summary of the violations found during the inspection are listed below.

3717-1-03.2(C) / Packaged and unpackaged food - preventing contamination by separation, packaging, and segregation
Critical Observed food that was not properly protected from contamination by separation, packaging, and segregation. Raw turkey burgers were stored over pork and beef in the walk-in at time of inspection.
To prevent contamination, food shall be protected from cross contamination by separating raw animal foods during storage, preparation, holding, and display from cooked ready-to-eat food as described in this rule. Foods shall be stored based on minimum cooking temperature (higher temp items shall be stored on bottom shelves).
3717-1-03.4(G) / Ready-to-eat, time/temperature controlled for safety food - date marking.
Critical Observed refrigerated, ready-to-eat, TCS foods (opened lettuce, cooked chicken, opened deli meat, baked potatoes, etc.) held refrigerated for more than 24 hours were not properly date marked at the time of inspection.
To prevent foodborne illness, refrigerated, ready-to-eat, TCS food held at a temperature of 41°F or less for more than 24 hours shall be clearly marked at the time of preparation or the time the original container is opened to indicate the date or day, that is a maximum of seven days, by which the food shall be consumed, sold, or discarded. The day the food is prepared or opened is day one. Per discussion with PIC, a new date marking system is being implemented for the FSO.
3717-1-05.1(J) / Backflow prevention device - when required.
Critical Observed no approved backflow prevention device on water connection to the Coffee brewing equipment at the time of inspection.
To prevent contamination, a plumbing system shall be installed to preclude backflow of a solid, liquid, or gas contaminant into the water supply system at each point of use at the FSO by installing an approved backflow prevention device as specified under paragraph (E) of this rule. Unless equipment has a backflow prevention device built-in to the equipment, it shall have a backflow prevention device (ASSE 1032) installed.

3717-1-02.4(A)(2) / Person in charge: assignment of responsibility.
Repeat Observed facility did not have one employee with supervisory and management responsibility and the authority to direct and control food preparation and service with level two certification in food protection at time of inspection (ODH certificate).
To prevent health hazards, obtain an ODH level 2 certificate in food protection. At least one employee that has supervisory and management responsibility and the authority to direct and control food preparation and service shall obtain the level two certification in food protection according to rule 3701-21-25 of the Administrative Code.
3717-1-02.4(C)(16) / Person in charge: duties - ensure the facility has written procedures for vomiting and diarrheal events.
Repeat Observed person-in-charge did not ensure that the facility has written procedures for employees to follow when responding to vomiting or diarrheal events at the time of inspection.
To ensure proper clean-up, person in charge shall ensure that the FSO have written procedures for employees to follow when responding to vomiting or diarrheal events that involved discharge onto surfaces. FSO shall develop or obtain a clean-up procedure.
3717-1-03.1(L) / Package integrity - specifications for receiving
Corrected During Inspection Observed food in packaging that does not appear to protect it from contamination. Observed multiple cans of food severely dented in the dry storage room at time of inspection.
To prevent adulteration, food packages shall be in good condition and protect the integrity of the contents so that the food is not exposed to adulteration or potential contaminants. The PIC voluntarily removed the cans.
3717-1-04.1(KK) / Food equipment - certification and classification.
Observed equipment (chest freezer) not approved by a recognized testing agency at the time of inspection.
To meet the requirements of this rule, food equipment that is acceptable for use in a FSO shall be approved by a recognized food equipment testing agency. Approved commercial equipment shall be used.
3717-1-05.1(S)(2) / Plumbing system - maintained in good repair.
Observed drainage line for the ice machine disconnected and leaking onto the floor at the time of inspection.
To prevent health hazards, a plumbing system shall be properly maintained. Repair plumbing system to be in good working order.

Repeat Observed the plumbing system for the warewashing sink was not properly maintained at the time of inspection. The drainage lines from the 3-compartment sink were leaking.
To prevent health hazards, a plumbing system shall be properly maintained. Repair plumbing system to be in good working order.
3717-1-06.2(E) / Handwashing signage.
Observed no handwashing sign posted at handwashing sink used by employees in the restroom at the time of inspection.
To remind employees to wash hands, a sign or poster that notifies food employees to wash their hands shall be provided at all handwashing sinks used by food employees. PIC stated extra hand signs are in the office. Provide a hand sign for the sink.
3717-1-06.4(A) / Repairing.
Observed the physical facilities are not being maintained in good repair. Water damaged ceiling (staining and peeling paint) was found above the warewashing/prep area at the time of inspection.
To prevent health hazards, the physical facilities shall be maintained in good repair. Repair the ceiling.
3717-1-06.4(J) / Using dressing rooms and lockers.
Corrected During Inspection Observed dressing rooms or lockers are not being properly used by employees to change clothes or store personal items at the time of inspection. Jacket, keys, cell phones, medication bottle were found on prep tables.
To prevent contamination, dressing rooms shall be used by employees if the employees regularly change their clothes in the establishment and lockers or other suitable facilities shall be used for the orderly storage of employee clothing and other possessions. Discontinue leaving personal items out on prep tables. Designated area for storing personal items shall be used.

All handwashing sinks shall be accessible at all times for employee use.
Food employees shall clean their hands and exposed portions of their arms before donning gloves to initiate a task that involves working with food; and after other activities that contaminate the hands.
