[?] A summary of the violations found during the inspection are listed below.

3717-1-04.8(E) / Equipment, utensils, linens, single-service articles, and single-use articles - storage.
Repeat Observed single-use articles (drink stirs on the bar) that were not properly stored at the time of inspection.
To prevent contamination, single-use articles shall be stored as specified under paragraph (E)(1) of this rule and shall be kept in the original protective package or stored by using other means that afford protection from contamination until used. Provide proper protection for the drink stirs to meet the requirements of this rule.
3717-1-06.2(E) / Handwashing signage.
Observed no handwashing sign posted at the handwashing sink used by employees in the basement restroom at the time of inspection.
To remind employees to wash hands, a sign or poster that notifies food employees to wash their hands shall be provided at all handwashing sinks used by food employees. Provide a sign at the hand sink.

The person in charge shall ensure that the FSO have written procedures for employees to follow when responding to vomiting or diarrheal events that involved discharge onto surfaces and also food.
A level 2 ODH certification in food safety training is required to be obtained for one operator of the facility by March 1, 2017.
All violations documented on this inspection will be checked for proper corrective action during next standard inspection.
