Critical Handwashing sink is being used for purposes other than handwashing. The employee was using the hand washing sink to rinse a cleaning rag. Education was provided to improve sink usage.

3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Repeat Corrected During Inspection Equipment food-contact surfaces or utensils are unclean (deli slicer). Education was provided to improve deli slicer cleaning procedures and the staff immediately cleaned the equipment once the violation was identified.

3717-1-03.4(G) / Ready-to-eat, time/temperature controlled for safety food - date marking.
Critical Refrigerated, ready-to-eat, TCS foods not properly date marked (ham, sausage, meatballs, rice). Education was provided to improve date marking procedures.

3717-1-03.5(C)(2)(e) / Food labels - label information
Critical Corrected During Inspection Missing and/or incomplete allergen declaration. Packaged rice and cole slaw were available for customer self-service without labels (ingredients, allergy information, address, etc.). The PIC immediately corrected the violation once it was identified by eliminating the customer self-service option.

A food packaged in the facility not labeled. Packaged rice and cole slaw were available for customer self-service without labels.

Repeat Food employees not wearing a hair restraint.

3717-1-03.2(G) / Washing fruits and vegetables.
Raw lettuce not washed prior to salad preparation.

3717-1-04.4(S)(1) / Single-service articles and single-use articles - use limitation.
Re-use of single-service articles. Cole slaw containers are being re-used to store tomatoes.

To-go containers were stored on the basement floor and in the mop closet.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Repeat Non-food contact surfaces of equipment are unclean (cooking equipment, cold holding equipment, dough equipment, storage racks and shelving).

3717-1-05.4(P) / Maintaining refuse areas and enclosures.
Repeat Improperly cleaned storage area for refuse, recyclables, or returnables. The dumpster is overflowing with trash and loose trash is scattered nearby.

3717-1-06.4(A) / Repairing.
Repeat Physical facilities not maintained in good repair. There are multiple areas throughout the kitchen that are in disrepair (flooring, walls, coved base, ceiling). Surfaces are no longer smooth, durable, and easily cleanable.

Repeat Facility not maintained clean. The floors are not clean in the kitchen, basement, and bathroom. Walls are not clean near food preparation areas.

Repeat Light intensity less than fifty foot candles in required food preparation areas.
