Critical PIC unable to demonstrate knowledge of major food allergens. Education was provided about allergy information and approved source information for cottage foods.

3717-1-02.4(B)(2)(m) / PIC: Demonstration of Knowledge - Identifcation and proper use of poisonous or toxic materials
Critical PIC unable to demonstrate knowledge of storage and use of toxic materials (residential pesticide). Education was provided to improve PIC knowledge of proper pesticide usage.

3717-1-02.4(C)(14) / PIC: duties - ensure employees are properly trained in food safety, including food allergy awareness.
Critical Person in charge did not ensure their employees are properly trained in food safety. PIC was not aware of proper ware washing procedures and pest control. Education was provided to improve food safety.

Critical Employees are not informed in a verifiable manner of their responsibility to report information about their health. There was no employee sick policy available. Education was provided about proper employee sick policy.

3717-1-03.1(A)(2) / Sources - home-produced foods
Critical Food prepared in a private home offered for human consumption. Unapproved muffins were offered to customers without following proper cottage food rules. Education was provided to the PIC about proper cottage food rules and the muffins were removed from sale.

3717-1-03.4(G) / Ready-to-eat, time/temperature controlled for safety food - date marking.
Critical Repeat Refrigerated, ready-to-eat, TCS foods not properly date marked (spinach and kale). Education was provided about proper date marking procedures.

3717-1-03.4(H)(1) / Ready-to-eat, time/temperature controlled for safety food - disposition.
Critical Corrected During Inspection Ready-to-eat, TCS food not properly discarded when required. Chicken with a preparation date of 8/22/21 was still being used in the kitchen eleven days beyond the proper discard date. Education was provided to improve date marking procedures and the PIC voluntarily discarded the chicken once the violation was identified.

Critical Improper use of a poisonous or toxic substance. A residential pesticide was stored in the FSO and not permitted. Education was provided to the PIC about proper pesticide treatments.

Critical Missing allergen declaration for the unlabeled packaged energy bites. Education was provided about proper label requirements and allergy information.

Critical Presence of a large population of live insects (flies) in the preparation area, especially near the juicer. Education was provided about proper pest control.

No towels or drying device at the handwashing sink near the ware washing area.

A food packaged in the facility not labeled. The packaged energy bites are not labeled with proper information (address, quantity, ingredients, etc.).

Outer opening not protected. Front doors were propped open during the entire inspection allowing for potential pest intrusion.

Corrected During Inspection Food employee not wearing proper hair restraint.

3717-1-03.2(G) / Washing fruits and vegetables.
Raw fruit (bananas) not washed, per PIC.

3717-1-03.2(K) / In-use utensils - between-use storage.
In-use utensils improperly stored. Ice scoops and frozen fruit scoops were stored with handles in contact with food.

3717-1-04.8(G)(1) / Kitchenware and tableware - handling of single-service and single use articles.
Corrected During Inspection Single-service spoons were displayed with handles going in the opposite direction allowing for contamination.

Exposed single service spoons were stored improperly allowing for potential customer contamination.
