[?] A summary of the violations found during the inspection are listed below.

3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Equipment food-contact surfaces or utensils are unclean. Observed deli slicer and knives at the prep table in the back room not clean to sight and touch. PIC stated employees will clean and sanitize the equipment and utensils before using.

3717-1-03.4(F)(1)(b) / Time/temperature controlled for safety food - cold holding.
Critical TCS foods not being cold held at the proper temperature. Observed multiple packaged items (mushrooms, bamboo shoots, seaweed, etc.) in the produce display cold holding unit that state "keep refrigerator" measured above 41 degrees F. PIC removed these items from sale. Provide documentation that products do not need to be held cold for food safety or ensure items are held under 41 degrees F.

3717-1-02.4(A)(2) / PIC - Level Two Certified Manager
Repeat Facility does not have an employee with level two certification in food protection.

3717-1-03 / Food - safe, unadulterated, and honestly presented
Observed frozen fish packaged by the facility showing signs of being thawed out and refrozen (ice crystals). PIC discarded the products.

3717-1-04.2(A) / Equipment - cooling, heating, and holding capacities.
Inadequate equipment available for cold holding food. Observed the produce display cold holding unit not holding TCS foods under 41 degrees F.

3717-1-03.5(C)(2) / Food labels - packaged food
Repeat Label on a food packaged in-house did not contain all required information. Observed labels on packages meat, fish, and vegetables that were missing the products common name, cut of meat, address, etc. Owner removed products without proper labeling and stated labeling machine is new and the owner is out for a few days. Ensure label is correct for all packaged products before displaying item for sale.

3717-1-06.1(M) / Outer openings - protected.
Repeat Observed the back exit door without proper protection against the entry of insects and rodents. The door was found not tight-fitting or able to self-close. Observed a wiping cloth stuffed in an exterior hole in the back storage closet.

3717-1-03.2(I)(2) / Storage or display of food in contact with water or ice - unpackaged foods
Unpackaged foods improperly stored/displayed in direct contact with undrained ice. Observed raw shrimp and other fish in containers with ice at the end of the long produce display cold holding unit.

3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Equipment components are not intact, tight or properly adjusted. Observed a middle freezer display door with excessive condensation and wet towels under the door. The area around the door is starting to build up an unknown black substance.

3717-1-05.1(S) / Plumbing system - maintained in good repair.
The plumbing system was not properly maintained. Observed a strong sewer odor in the back room, mop room, and restroom at the time of inspection. Please provide documentation when grease trap has been cleaned.

3717-1-05.4(P) / Maintaining refuse areas and enclosures.
Improperly cleaned storage area for refuse, recyclables, or returnables. Observed items, boxes, and other debris behind the FSO that are not placed in the outdoor receptacles at the time of inspection.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Observed floors in the back prep, walk-in units, dry storage area, and self-service bulk food area not maintained clean.

3717-1-06.2(I)(3) / Lighting - intensity (50 FC)
Light intensity less than fifty foot candles in required areas. Observed 35-40 foot candles at the prep table where the deli slicer and knives are used.
