3717-1-06.1(M) / Outer openings - protected.
Repeat Outer opening not protected. Observed front door unable to latch, causing door to always be slightly open. Observed self-closing device for the back door in disrepair.

3717-1-06.4(A) / Repairing.
Physical facilities not maintained in good repair. Observed damaged ceiling in the warewashing room above the shelving units. Damaged covebase\flooring in the bathroom, damaged trim pieces around opening near the office desk.

Comments
Ensure the drainage line from the condensing unit in the walk-in is directed into a floor drain with an air gap.
Floors, walls, and ceiling shall be smooth and easily cleanable. Any damages in the facility shall be repaired to allow for cleaning.
Sinks shall be secured and sealed to the walls.
Ensure new employees are reviewing and signing the FSO's sick policy (employee health reporting agreement).

The violations documented are to be corrected and will be checked during next standard inspection.
