3717-1-02.4(C)(16) / PIC: duties - ensure written procedures and plans are maintained and implemented in the facility.
Critical Corrected During Inspection Written procedures and plans are not maintained and implemented. Observed open bags of steak, ham, and chicken in the prep refrigeration unit without a date mark. PIC stated they have a carry-over prep process that they follow and the bag should be fully emptied into a container and used within the day. Observed no time mark on the pizza sauce at the prep table. Employee placed time mark on the product. PIC stated they will ensure employees are following the carry over procedure and the time marking procedue for the sauce.

3717-1-02.4(A)(2) / PIC - Manager Certification
Repeat Facility does not have an employee with manager certification in food protection.

3717-1-06.1(M) / Outer openings - protected.
Repeat Outer opening not protected. Observed front door unable to latch, causing door to always be slightly open.

3717-1-03.2(K) / In-use utensils - between-use storage.
In-use utensils improperly stored. Observed pizza slicer stored on cardboard at the prep table. Utensils shall be stored on the food prep table only if the utensil and food contact surface are cleaned and sanitized every 4 hours. PIC stated the cardboard is switched every 2 hours with the utensils.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Observed accumulation of food residue and debris on top the pizza oven, on spray nozzle for three-compartment sink, and on the scale at prep refigeration unit.

Ensure the drainage line from the condensing unit in the walk-in is directed into a floor drain with an air gap.
Floors, walls, and ceiling shall be smooth and easily cleanable. Repairs shall be made if found to no longer be smooth and easily cleanable.
Sinks shall be secured and sealed to the walls.

The violations documented shall be corrected and will be checked during the follow-up inspection.
