Comments
This pre-licensing inspection was completed per the request from the owner to have the Knockdown MFSO issued a license. The MFSO has been approved to be issued a license with the understanding the following will be completed:

1) The hot dog roller grill is not NSF or similar testing agency approved for commercial use. It may be used for the June 12 event but shall be replaced with a NSF or equivalent testing agency grill by July 30.
2) Non-commercial refrigerator (back labeled Household listed) may be used for this licensing year only and shall be replaced with NSF or equivalent testing agency unit before renewal in 2022.
3) Repair the leaks in the water lines inside the sink cabinet.
4) Provide a working canopy/tent/overhead protection. This required to be used at all times (unless in a pavilion).
5) Provide a Time instead of temperature procedure if scoops will be stored in water with out cold or hot holding. Handout provided in inspection email.
6) Illness training verification will be maintained at the FSO location. Recommend providing copies with the mobile unit.
7) Provide processing or warehouse licensing information by June 30.
8) Shatter proof lighting shall be provide for night/evening operations.
9) Provide a 10 pound ABC fire extinguisher per Ohio fire code. See Kent Fire Dept. handout in inspection email.
10) Provide documentation that at least one person with supervisor duties is PIC certified will be working the MFSO at all times.
11) A vomit and diarrheal clean up kit with proper procedure should be provided.
12) Replace current backflow prevent device with a Lead Free ASSE 1024 device. Current is not LF (Lead-free).
13) Submit a signed MFSO license applications and proper payment of $310.00.

All items on this inspection report will be rechecked at the follow-up inspection on June 30. This is not a final list and subject to change upon further review of the MFSO.
