[?] A summary of the violations found during the inspection are listed below.

3717-1-04.8(E) / Equipment, utensils, linens, single-service articles, and single-use articles - storage.
Repeat Observed cleaned equipment and utensils, laundered linens, or single-service and single-use articles are not properly stored at the time of inspection. Multiple items were found stored on the floor throughout the facility.
To allow for proper cleaning and prevent pest harborage, cleaned equipment and utensils, laundered linens, single-service articles, and single-use articles shall be properly stored: (a) In a clean, dry location; (b) Where they are not exposed to splash, dust, or other contamination; and (c) At least six inches (fifteen centimeters) above the floor unless the items are kept in closed packages on dollies, pallets, racks, and skids that are designed as specified under paragraph (II) of rule 3717-1-04.1 of this code. Discontinue storing items on floors.
3717-1-04.8(G) / Kitchenware and tableware - handling.
Repeat Observed single-service or single-use articles were not handled, displayed, or dispensed properly. Multiple stacks of single use cups were found unprotectected on top of the customer self-service counter (near pop machine) at the time of inspection.
To prevent contamination, single-service articles or single-use articles and cleaned and sanitized utensils shall be handled, displayed, and dispensed so that contamination of food-contact-and lip-contact surfaces is prevented as described in this rule. Discontinue stacking single use articles unprotected on the customer self service counters.
3717-1-05.1(S)(1) / Plumbing system - maintained in good repair.
Repeat Observed the plumbing system for the hand sink in the restroom was not properly repaired. Drainage line from sink was leaking at the time of inspection.
To prevent health hazards, a plumbing system shall be repaired according to the Ohio building code. Repair hand sink to be in good working order.
3717-1-06.4(A) / Repairing.
Repeat The physical facilities are not being maintained in good repair. Observed a damaged self-closing mechanism for the back exit door at the time of inspection.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Observed a build-up of dirt and debris on the floors in the following location at the time of inspection: (1) hallway around the ice machine (2) behind the front service counter (3) walk-in cold holding unit (liquid spills) (4) along the permiter throughout the facility. (5) all other hard to reach areas.
To prevent health hazards, the physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed. The areas listed above shall be cleaned and maintained. Boxes shall be stored off the floor to allow for easy cleaning.
3717-1-06.4(F) / Drying mops.
Observed mop being dried using an unapproved method. Soiled mop was observed stored on the floor next to the mop sink and not being dried at the time of inspection.
To prevent attracting pests, after use, mops shall be placed in a position that allows them to air-dry without soiling walls, equipment, or supplies. Discontinue storing soiled mops next to the mop sink, preventing the mop from drying.

The equipment in the back room is currently not in use.
Per discussion with manager, the health department will be contacted when the RFE expands the operation.
The violations documented shall be corrected as soon as possible and will be checked during next standard inspection.
