3717-1-02.4(A)(2) / PIC - Manager Certification
Repeat Facility does not have an employee with manager certification in food protection.

3717-1-03.2(K) / In-use utensils - between-use storage.
Repeat In-use utensils improperly stored. Observed Falafel press utensils were stored in room temperature water between uses. If the press is to be stored in water between uses, it shall stored in hot water that is held at 135 degrees F or higher or with in a working dipper well.

3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Repeat Observed tall reach-in freezer to have damaged gaskets and ice accumulation. Operator is not using the equipment due to the icing issue. Equipment components are not intact, tight or properly adjusted.

3717-1-04.1(KK) / Food equipment - certification and classification.
Repeat Observed multiple equipment items not approved by a recognized testing agency. Remove the unapproved microwave, blenders, household vacuum sealer, salad spinner, household container, lowes buckets, High point freezer, reuse of glass jelly jars, and any other household use items.

Repeat Observed Nu Wave oven to have a damaged gasket. Equipment components are not intact, tight or properly adjusted. Replace gasket.

Repeat Observed plastic seals and door seals on preparation cold holding equipment in need of replacement/repair. If replacement of parts is not available then the equipment shall be replaced. Equipment components are not intact, tight or properly adjusted.

3717-1-06.4(A) / Repairing.
Repeat Observed grout between floor tiles in the warewashing area of the back kitchen to be in need of repair. Physical facilities not maintained in good repair.

3717-1-06.2(I)(3) / Lighting - intensity (50 FC)
Repeat Light intensity less than fifty foot candles in required areas (all food preparation, cooking, warewashing areas).

Repeat Light intensity less than twenty foot candles in required areas (hand washing sinks).

3717-1-02.4(B)(2)(f)/PIC: Demonstration of Knowledge - Stating required temperatures and times for safe cooking of TCS food
PIC unable to demonstrate knowledge of food temperatures and times for safe cooking when asked. Review proper cooking temperatures and train all cooking staff.
3717-1-03.4(F)(1)(b)/Time/temperature controlled for safety food - cold holding.
Observed multiple TCS foods (in house made tahini sauce (65 degrees F), other in house made sauces, garlic sauce (53 degrees F), cooked onions (room temp), not being cold held at the proper temperature. Cold TCS foods shall be held a 41 degrees or below. Hot TCS foods shall be held at 135 degrees F or higher. Foods were not discarded at the time of inspection. A Time instead of temperature procedure is required before TCS foods can be held with cold holding.
3717-1-03.4(H)(1)/Ready-to-eat, time/temperature controlled for safety food - disposition.
Observed Ready-to-eat, TCS food (TCS baked items 10/7, kibbeh, Humus 10/10 etc.) were not properly discarded when required. Foods shall be properly discarded 7 days after preparation or opening from original packaged. Foods were not discarded at the time of inspection.
3717-1-03.4(G)/Ready-to-eat, time/temperature controlled for safety food - date marking.
Observed multiple refrigerated, ready-to-eat, TCS foods (falafel, tahini sauce, other in house made sauces, cut tomatoes mix, etc) not properly date marked. PIC knowledge regarding TCS foods needs improvement. Foods were not discarded. If foods are not proper date marked at the follow-up inspection foods may be embargoed.
3717-1-03.5(C)(2)/Food labels - packaged food
Label on a food packaged in-house did not contain all required information. Food pre-packaged in facility (humus) shall be properly labeled.
3717-1-03.2(D)/Food storage containers - identified with common name of food.
Working food containers not properly labeled with common name of foods.
3717-1-06.4(A)/Repairing.
Observed the walk-in cooler condensation drainage line was leaking on to food boxes, food rack and floor. Physical facilities not maintained in good repair. Discontinue storing any thing under the leaking condensation drainage lines. Food items leaked on shall be considered soiled and discarded. No foods were discarded at the time of inspection.
3717-1-06.4(B)/Cleaning - frequency and restrictions.
Facility not maintained clean. Observed large amount of oil on the floors under and behind the hot line. PIC stated they did not know were the oil came from. Clean oil and food spills quickly to prevent larger cleaning issues. If fryer equipment is leaking, repair equipment.
3717-1-04.1(C)/Food-contact surfaces - cleanability
Food contact surfaces not easily cleanable. Observed Lowes buckets, reuse of glass jars, reuse of plastic food containers, and household use containers. These items are not considered easily cleanable or are not designed for re-use. Discontinue using these items.
3717-1-02.4(B)(1)/PIC: Demonstration of Knowledge - No Critical Violations
PIC unable to demonstrate knowledge by having no critical violations.
Comments
All critical violations were observed to be corrected. Continue to work on processes and procedures to ensure compliance with the Ohio Food Code. Critical violation compliance is required to avoid additional enforcement action from licensor.

Lighting and other facility issues are in the process of correction by the landlord.

Recommend the PIC attend the KCHD certified managers course to help with knowledge and to obtain the required ODH certification.

All violations will be rechecked at the next standard inspection.
