| This inspection was completed per the request from owner to be issued a licensed for 2021. Since the MRFE did not renew in 2020 due to the pandemic, the old license/facility was place Out of Business and a new license will be issued. The MRFE did not go out of business but was not able to operate due the safety issues with the Pandemic. No violations were documented at the time of inspection. The MRFE has been approved to be issued a 2021 MRFE license. Nothing has changed on the mobile bicycle cart or with the Cottage Food operation. Okay to Operate. Check# 1016 for $310 and signed application for license number JSMH-BXTJUC was received at the time of inspection. Restrictions of the MRFE will be printed on the back of the license. Provide approved hand sanitizer wipes. If MRFE operates after sunset, ensure proper shatter proof lighting is provided. Complete ingredient book must be provided at all times of operation. License will be mailed once processed. Notes: There are plans for a future bakery location. Recommend checking with ODA regarding bakery and wholesale license requirements. |
