3717-1-02.4(C)(14) / Person in charge: duties - ensure employees are informed in a verifiable manner of their responsibility to report their health information
Critical Person in charge did not ensure that employees are informed in a verifiable manner of their responsibility to report information about their health. Observed the FSO without signed employee reporting agreements at time of inspection.
To prevent transmission of foodborne illness, the person in charge shall ensure that food and conditional employees are informed in a verifiable manner of their responsibility to report information about their health as it relates to diseases that are transmissible through food.

3701-21-25(I) OAC / Level one certification in food protection
The FSO did not have a person in charge that had completed a Level One Certification course. Observed the FSO without a food safety basics certificate at time of inspection.
To prevent health hazards, provide training to all shift managers.
3717-1-02.4(A)(2) / Person in charge: assignment of responsibility.
Observed the facility did not have one employee with supervisory and management responsibility and the authority to direct and control food preparation and service with level two certification in food protection at time of inspection.
To prevent health hazards, at least one employee that has supervisory and management responsibility and the authority to direct and control food preparation and service shall obtain the level two certification in food protection according to rule 3701-21-25 of the Administrative Code.
3717-1-03.5(C)(2) / Food labels - packaged food
A label on a food packaged in the RFE or FSO did not contain all required information. Observed baked goods from Busy B Bakery improperly labeled at time of inspection.
To prevent health hazards, provide proper labeling for all baked goods. Foods shall be labeled as specified in this rule.
3717-1-04.1(KK) / Food equipment - certification and classification.
Equipment is not approved by a recognized testing agency. Observed non-commercial food storage containers at time of inspection.
To prevent health hazards, provide commercial food grade storage containers. Food equipment that is acceptable for use in a FSO or RFE shall be approved by a recognized food equipment testing agency.
3717-1-06.4(A) / Repairing.
The physical facilities are not being maintained in good repair. Observed the main hand washing sink with a leaking spigot at time of inspection.
To prevent health hazards, repair the sink. The physical facilities shall be maintained in good repair.
