[?] A summary of the violations found during the inspection are listed below.

3717-1-03.2(J) / Food contact with equipment and utensils
Critical Observed the container used for transporting ice to the ice bin connected to the soda dispenser was being used for other tasks at the time of inspection.
Food shall only contact the surfaces of cleaned and sanitized equipment and utensils or single-use and/or single-service articles. Provide a new container and label the container "Ice Only". This container shall only be used for ice and no other tasks to limit possible contamination to the ice.
3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Observed slight accumulation of unknown black substance in the interior of the ice machine at the time of inspection. Equipment food-contact surfaces or utensils are dirty.
To prevent contamination, equipment food-contact surfaces and utensils shall be clean to sight and touch. Clean ice machine and maintain the interior free of soil accumulation.
3717-1-05.1(J) / Backflow prevention device - when required.
Critical Observed no approved backflow prevention device on the plumbing system to the soda dispensing equipment, coffee brew machines, and icee machine at the time of inspection.
A plumbing system shall be installed to preclude backflow of a solid, liquid, or gas contaminant into the water supply system of use at the FSO or RFE, by providing an air gap as specified under this rule, or installing an approved backflow prevention device as specified under this rule. Provide proper back flow prevention on water connections to all equipment described above.
3717-1-05.1(S)(1) / Plumbing system - maintained in good repair.
Critical Observed the plumbing system located at the 3 compartment sink was not properly repaired at the time of inspection.
A plumbing system shall be repaired according to the Ohio building code. Repair the faucet fixture on the 3 compartment sink to allow for proper ware washing of utensils and equipment.
3717-1-05.3(C) / Backflow prevention.
Critical Observed a direct connection between the sewage system and a drain originating from equipment in which food (the ice bin on the soda dispenser machine), portable equipment, or utensils are placed.
A direct connection may not exist between the sewage system and a drain originating from equipment in which food, portable equipment, or utensils are placed unless allowed by the Ohio building code. To prevent contamination to ice and foods, provide a air gap on the drainage line.

3717-1-04.5(D) / Nonfood-contact surfaces - cleaning frequency.
Observed accumulation of soil residue on nonfood-contact surfaces. Observed dust build-up on the condenser fan grate at the tome of inspection
Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues. Clean the condenser fan cover and maintain free of dust accumulation.
3717-1-06(A)(1) / Indoor areas - surface characteristics
Observed wall that were not constructed of approved materials behind the soda dispenser and smoothie machine at the time of inspection.
Where FSO or RFE activities are conducted, materials for indoor floor, wall, and ceiling surfaces shall be smooth, durable, and easily cleanable; and nonabsorbent for areas subject to moisture. Install a smooth, durable and easily cleanable surface behind the soda dispenser machine. This will help maintain the facility in good repair and allow for the wall to be easily cleaned.
3717-1-06.1(M) / Outer openings - protected.
Observed an outer opening, window, or door without proper protection against the entry of insects and rodents. Observed the front door was propped open at the time of inspection
To prevent contamination, outer openings of an FSO or RFE shall be protected against the entry of insects and rodents as specified in this rule; a) Filling or closing holes and other gaps along floors, walls, and ceilings; (b) Closed, tight-fitting windows; and (c) Solid, self-closing, tight-fitting doors; (2) Exterior emergency exit doors need not be self-closing if they are: (a) Solid and tight-fitting; (b) Designated for use only when an emergency exists, by the fire protection authority that has jurisdiction over the FSO or RFE; and (c) Limited-use so they are not used for entrance or exit from the building for purposes other than the designated emergency exit use; (3) If the windows or doors of an FSO or RFE, or of a larger structure within which the facility is located, are kept open for ventilation or other purposes or a temporary FSO or temporary RFE is not provided with windows and doors as specified under paragraph (M)(1) of this rule, the openings shall be protected against the entry of insects and rodents by: (a) Sixteen mesh to one inch (sixteen mesh to 25.4 millimeters) screens; (b) Properly designed and installed air curtains to control flying insects; or (c) Other effective means.
3717-1-06.4(A) / Repairing.
Observed the coved base molding be in need of a repairs at the time of inspection. The physical facilities are not being maintained in good repair.
The physical facilities shall be maintained in good repair. Repair the coved base molding in the back room.
3717-1-06.4(N) / Maintaining premises - unnecessary items and litter.
Observed unnecessary or nonfunctional items and /or litter on the premises in the back room at the time of inspection
The premises shall be free of items that are unnecessary to the operation or maintenance of the FSO or RFE such as equipment that is nonfunctional or no longer used and litter. Organize the back room by using selves and dunnage racks. All equipment that is not needed shall be removed from the RFE premises.
