3717-1-03.2(C) / Packaged and unpackaged food - preventing contamination by separation, packaging, and segregation
Critical Corrected During Inspection Observed food that was not properly protected from contamination by separation, packaging, and segregation. Observed raw pork was stored in the walk-in cold holding unit directly over and next to raw produce at the time of inspection.
To prevent contamination, food shall be protected from cross contamination by separating raw animal foods during storage, preparation, holding, and display from cooked ready-to-eat food. Raw foods shall not be stored over produce or in the same container as produce.
3717-1-03.4(F)(1) / Time/temperature controlled for safety food - hot and cold holding.
Critical Corrected During Inspection Observed TCS foods (tofu, Lotus slices) were not being held at the proper temperature at the time of inspection.
To prevent the growth of pathogens, except during preparation, cooking, or cooling, or when time is used as a public health control, TCS food shall be held at 135°F or above (except that roasts cooked using time and temperature parameters in 3717-1-03.3 may be held at 130° F), or at 41°F or less. Packages that state "Keep Refrigerated" shall be held at temperatures that meet the requirements of this rule. Foods were removed from the sales shelf at the time of inspection.

3717-1-03.5(C)(2) / Food labels - packaged food
A label on a food packaged in the RFE did not contain all required information. Observed packages of frozen raw meats, fish and produce without proper labels at the time of inspection.
Food label information shall include: (a) The common name of the food, or an adequately descriptive identity statement; (b) If made from two or more ingredients, a list of ingredients in descending order of predominance by weight, including a declaration of artificial color or flavor and chemical preservatives, if contained in the food; (c) An accurate declaration of the quantity of contents; (d) The name and place of business of the manufacturer, packer, or distributor; (e) The name of the food source for each major food allergen contained in the food, unless listed in the ingredients; (f) Nutritional information when required by law; and (g) Proper Labeling on container of salmonoid fish containing canthaxanthin.
3717-1-03.5(C)(3) / Food labels - bulk food
Observed Bulk food display (produce) for customer self-service was not properly labeled.
Bulk food that is available for consumer self-dispensing shall be prominently labeled with the following information in plain view of the consumer: (a) the manufacturer's or processor's label that was provided with the food, or (b) a card, sign, or other method that includes the information specified under (C)(2) of this rule.

A Complaint was received regarding foods with packages stating "Keep Refrigerated" were warm to the touch and that many foods were not provided with proper labels.
See violations regarding this complaint.
Violations will be re-checked at the 30 day inspection.
Operator was willing to pull all items that were found in violation.

Please provide documentation the mushrooms being sold at this location are from an approved source.
