| This inspection was completed per the request from owner to be issued a licensed for 2022. No violations were documented at the time of inspection. The MRFE has been approved to be issued a 2022 MRFE license. Observed proper labels, sign, and set-up at the time of inspection. Okay to Operate. Check# 99 for $310 and signed application for license number JSMH-CGZNLK was received at the time of inspection. Restrictions of the MRFE will be printed on the back of the license. No hand washing sink is required. Recommend providing NSF approved hand wipes. Foods to be sold are limited to Cottage foods that have been manufactured and labeled according to the Ohio Cottage Food Rules If MRFE operates after sunset, ensure proper shatter proof lighting is provided. Complete ingredient book must be provided at all times of operation. License will be mailed once processed. |
