901:3-4-07.0 / Facility layout and equipment specifications for RFE
RFE constructed, altered, and/or equipment added without prior approval from the licensor. Observed shelving, front counter, back cafe customer self-service line, cold holding equipment have been removed or altered. Observation were made through window as the RFE was closed at the time of inspection.

Comments
During previous conversations with operator, they stated that the RFE will be converting to a FSO and provide risk level 3 or 4 services. The current layout and facilities may need significant changes to accommodate FSO operations. Therefore, a complete plan review will be required by the Licensor. Considered this inspection as a notice to Cease demolition and altering the facility until approval as been provided in writing. Approval will be provide once a completed Food Facility Plan Review is submitted and all required documentation is provided.
