Critical Repeat Person in charge did not ensure their employees are properly trained in food safety. The PIC lacked proper food safety knowledge (date marking, source, labels, sick policy, etc.). Education was provided to improve food safety.

3717-1-02.4(C)(15) / PIC: duties - ensure employees are informed in a verifiable manner of their responsibility to report their health information
Critical Repeat Corrected During Inspection Employees are not informed in a verifiable manner of their responsibility to report information about their health. The proper employee sick policy was not available until the violation was identified and provided from the inspector for employees to sign at time of inspection.

3717-1-03.1(A)(2) / Sources - home-produced foods
Critical Corrected During Inspection Food prepared in a private home used or offered for human consumption (chicken). PIC stated that chicken was prepared at home and used for chicken wraps. Education was provided about proper source and the chicken was voluntarily discarded.

3717-1-03.4(G) / Ready-to-eat, time/temperature controlled for safety food - date marking.
Critical Refrigerated, ready-to-eat, TCS foods not properly date marked (chicken). Education was provided to improve date marking procedures.

3717-1-03.5(C)(2)(e) / Food labels - label information
Critical Repeat Corrected During Inspection Missing and/or incomplete allergen declaration. Packaged home baked cookies did not contain a proper label. Education was provided about proper labels, cottage foods, and the item was removed from sale.

3717-1-05.3(C) / Sewage and other liquid waste: Backflow prevention.
Critical Corrected During Inspection Direct connection between the sewage system and a drain originating from equipment in which food, portable equipment, or utensils are placed. The drain to the ice machine did not have a proper air gap. The PIC was able to adjust the drain once the violation was identified.
