| This pre-licensing inspection was completed per the request from the owner to have the Knockdown MFSO issued a license. The MFSO has been approved to be issued a license with the understanding the following will be completed: 1) Illness training verification will be maintained at the FSO location. Recommend providing copies with the mobile unit. 2) Provide documentation that at least one person with supervisor duties is PIC certified will be working the MFSO at all times. 3) A vomit and diarrheal clean up kit with proper procedure should be provided. 4) Submit a signed MFSO license applications and proper payment of $330.00 before operating Okay to Operate once application and fees are submitted to the KCHD and items listed above have been address. No violations were documented at the time of inspection. License will be mailed to the designated mailing address on application. |
