3717-1-03.2(J) / Food contact with equipment and utensils
Critical Observed food (ice) in contact with an unclean surface (beverage container used as scoop) at the time of inspection.
To prevent contamination, food shall only contact the surfaces of cleaned and sanitized equipment and utensils or single-use and/or single-service articles. Do not use beverage containers to serve or scoop ice. Provide a clean and sanitized scoop for each location where ice is served. Correct By: 12-Aug-2015
3717-1-03.4(H)(1) / Ready-to-eat, time/temperature controlled for safety food - disposition.
Critical Corrected During Inspection Observed ready-to-eat, TCS food (chicken noodle soup and hamburger sauce) that had been date marked was not properly discarded when required at the time of inspection.
To prevent illness, refrigerated, ready-to-eat TCS food that has been date marked in the facility shall be discarded if its "use by" date has expired. The person-in-charge discarded the chicken noodle soup and hamburger sauce.
3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Observed food-contact surfaces of the deli slicer were dirty at the time of inspection.
To prevent contamination, equipment food-contact surfaces and utensils shall be clean to sight and touch. Remove all food debris has accumulated in the grooves and openings of the deli slicer, then clean and sanitize all parts of this equipment. Correct By: 12-Aug-2015 Correct By: 12-Aug-2015
3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Observed dried blue paint an an accumulation of dried food debris on the drain board of the 2-compartment sink at the time of inspection.
To prevent contamination, equipment food-contact surfaces and utensils shall be clean to sight and touch. Clean the paint and food debris from the 2-compartment sink. Correct By: 12-Aug-2015

3717-1-04.1(KK) / Food equipment - certification and classification.
Repeat Observed deli slicing equipment is not approved by a recognized testing agency (slicer is very old) at the time of inspection.
To prevent contamination to foods, food equipment that is acceptable for use in a FSO shall be approved by a recognized food equipment testing agency. Replace deli slicer with a new and approved (by NSF or similar testing agency) equipment once the current equipment is no longer in proper working order.
3717-1-04.1(Y) / Temperature measuring devices.
Observed cold holding equipment (salad bar fridge) did not contain a thermometer at the time of inspection.
To ensure that TCS foods held cold are maintained at a temperature that limits the growth of harmful organisms, a properly designed temperature measuring device shall be located in the warmest part of a cooling unit. Provide a properly located thermometer for this equipment.
3717-1-04.4(S)(1) / Single-service articles and single-use articles - use limitation.
Observed re-use of single-service portion container that was used as a scoop for coleslaw at the time of inspection.
To prevent contamination, single-service and single-use articles may not be reused. Portion containers that are used as scoops shall be discarded after one use.
3717-1-04.8(E) / Equipment, utensils, linens, single-service articles, and single-use articles - storage.
Observed single-use articles (paper towels on the floor of the dry storage room) were not properly stored at the time of inspection.
To prevent contamination, cleaned equipment and utensils, laundered linens, single-service articles, and single-use articles shall be properly stored in a clean, dry location where they are not exposed to splash, dust, or other contamination; and at least six inches (fifteen centimeters) above the floor. Move the paper towels to an approved location.
3717-1-05.3(I) / Other liquid wastes and rainwater.
Repeat Observed improper draining of condensation and other nonsewage liquids from the walk-in cold and freezer equipment at the time of inspection.
Condensate drainage and other nonsewage liquids and rainwater shall be drained from point of discharge to disposal according to law. Condenser units shall be draining to a drain that flows to a sanitary sewer. Reinstall the drainage of the walk-in units to meet the requirement of this rule.
3717-1-06.4(A) / Repairing.
Observed damaged floor tiles in front of the walk-in cooler at the time of inspection.
So that floors may be adequately cleaned, the physical facilities shall be maintained in good repair. Repair the damaged flooring in front of the walk-in cooler.
