Repeat Facility does not have an employee with manager certification in food protection.

3717-1-06.4(N) / Maintaining premises - unnecessary items and litter.
Repeat Unnecessary or nonfunctional items and /or litteron the premises. Observed equipment under tarps on the side of the building, litter behind the building, and damaged products and equipment by the back door.

901:3-4-16(C) / Person in Charge Certification in Food Protection
Repeat RFE does not comply with Person in Charge Certification requirements.

901:3-4-07.0 / Facility layout and equipment specifications for RFE
RFE constructed, altered, and/or equipment added without prior approval from the licensor. Observed an extention to the walk-in refrigeration unit and a doorway installed to have two entrances into the walk-in unit. This additional walk-in space was previously the restrooms, that were in disrepair and not used.

Observed the kitchen area no longer in use and being used as dry storage. Kitchen is closed and PIC states they no plans to operate this section of the store. .
Ensure equipment that is no longer used in the facility is properly stored/removed from the facility.

The facility may request a decrease in risk level for the next licensing period based on discussion with PIC.
A drop in risk category for your upcoming license will be attached with the inspection report.
Please submit a facility review packet to the Health Department for the addition walk-in space as soon as possible.
