Comments
This RFE has undergone a final pre-licensing inspection and has been conditionally approved to be issued a RFE Risk Class III license with the understanding the following will be completed:

1) Provide proper coved finish on all floor and wall junctures.
2) All shelving shall be installed 6 inches up off the floor or installed with casters to be movable. This includes the shelves in basement and Dairy crates under pizza boxes.
3) An acceptable vomit and diarrheal clean up kit with proper procedure has been provided. Ensure staff is aware and knowledgeable of the procedures.
4) Provide a written agreement regarding the use of Zephyrs outdoor refuse storage and waste hauler contract.
5) Clean floors and walls in hard to reach locations.
6) Provide a proper transition cover from the white tile and blue tile.
7) Provide information on the new equipment to be installed at the mop sink. Ensure system is properly backflow prevented.
8) Remove the old unused freezers in the basement.
9) Provide proper wall covers (FRP or similar) around the mop sink area, dough preparation corner and behind the be food prep sink (tile is not sticking).
10) Remove the hotshot hanging pest controlling stations. These are not permitted for using a RFE/FSO per the pesticide law and label.

If above items are present at the 30 day inspection, the inspector may violate according to the Ohio Food Code. Significant improvements were made allowing for the conditional approval. Failure to continue to improve may lead to additional enforcement action from the KCHD.

A Signed application and proper license fee was submitted. Okay to Operate.
