3717-1-02.4(C)(14) / Person in charge: duties - ensure employees are informed in a verifiable manner of their responsibility to report their health information
Critical Repeat Person in charge did not ensure that employees are informed in a verifiable manner of their responsibility to report information about their health. Observed the FSO without properly signed employee reporting agreements at time of inspection.
To prevent transmission of foodborne illness, the person in charge shall ensure that food and conditional employees are informed in a verifiable manner of their responsibility to report information about their health as it relates to diseases that are transmissible through food.
3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Equipment food-contact surfaces or utensils are dirty. Observed the interior surfaces of the ice machine to be dirty at time of inspection.
To prevent contamination, clean the ice machine. Equipment food-contact surfaces and utensils shall be clean to sight and touch.

3701-21-25(I) OAC / Level one certification in food protection
Repeat The FSO did not have a person in charge that had completed a Level One Certification course. Observed the PIC without a food safety basics certification at time of inspection.
To prevent health hazards, provide training to all shift managers.
3717-1-03.5(C)(2) / Food labels - packaged food
Repeat A label on a food packaged in the RFE or FSO did not contain all required information. Observed packages of chocolate covered espresso beans without labels at time of inspection.
To prevent health hazards, provide labels for all packaged food. Foods shall be labeled as specified in this rule.
3717-1-04.8(E) / Equipment, utensils, linens, single-service articles, and single-use articles - storage.
Cleaned equipment and utensils, laundered linens, or single-service and single-use articles are not properly stored. Observed cups stored on the floor at time of inspection.
To prevent contamination, store cups at least 6 inches from the floor. Cleaned equipment and utensils, laundered linens, single-service articles, and single-use articles shall be properly stored as described in this rule.
3717-1-06.4(A) / Repairing.
The physical facilities are not being maintained in good repair. Observed multiple areas of the facility in disrepair at time of inspection: the screen door was in disrepair; the ware washing sink drain was leaking; and the hand washing sink had a leaking spigot.
To prevent health hazards, repair the facility. The physical facilities shall be maintained in good repair.
