3717-1-02.4(C)(14) / Person in charge: duties - ensure employees are informed in a verifiable manner of their responsibility to report their health information
Critical Person in charge did not ensure that employees are informed in a verifiable manner of their responsibility to report information about their health.
To prevent transmission of foodborne illness, the person in charge shall ensure that food and conditional employees are informed in a verifiable manner of their responsibility to report information about their health as it relates to diseases that are transmissible through food. A Handout was given to the owner to implement a policy asap.
3717-1-03.2(C) / Packaged and unpackaged food - preventing contamination by separation, packaging, and segregation
Critical Corrected During Inspection Observed food that was not properly protected from contamination by separation, packaging, and segregation at the time of inspection. Raw beef and fish were being stored below raw chicken.
To prevent contamination, food shall be protected from cross contamination by separating raw animal foods during storage, preparation, holding, and display from cooked ready-to-eat food as described in this rule. Discontinue storing beef and fish below raw chicken. Owner corrected the issue and designated locations for the meats.

3717-1-04.1(KK) / Food equipment - certification and classification.
Observed equipment (2 chest freezers, coffee maker) that was not approved by a recognized testing agency at the time of inspection.
To meet the requirements of this rule, food equipment that is acceptable for use in a FSO shall be approved by a recognized food equipment testing agency. Replace "household use only" equipment with NSF approved equipment or from a similar testing agency.
3717-1-05.4(O) / Using drain plugs.
Observed the outside waste receptacle drain that did not have plug at the time of inspection.
To prevent attracting pests, drains in receptacles and waste handling units for refuse, recyclables, or returnables shall have drain plugs in place. Provide drain plug for the outside receptacle. Recommend contacting the facility's waste hauler.
3717-1-06.1(A) / Floors, walls, and ceilings.
Observed floors and walls not smooth and easily cleanable under and around the bar at the time of inspection.
To prevent contamination, the floors, floor coverings, walls, and wall coverings shall be designed, constructed, and installed so they are smooth and easily cleanable. The under/behind the bar area shall be provided with backboards and cove base constructed of material that meets the requirements of this rule (FRP, stainless steel, tile or similar material).
3717-1-06.4(A) / Repairing.
Observed shelving units in the walk-in units in poor condition at the time of inspection.
To prevent health hazards, the physical facilities shall be maintained in good repair. Repair or replace damaged shelving units.
3717-1-06.4(B) / Cleaning - frequency and restrictions.
Observed a build-up of dirt and debris on the floors in the following locations at the time of inspection: (1) walk-in cold holding unit. (2) walk-in freezer. (3) behind the bar. (4) near grease trap.
To prevent health hazards, the physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed. The areas listed above shall be cleaned and maintained.

If equipment is used only for employees then provide a label stating that the food/equipment is only used for employees personal use.
Ensure food prep sink is being cleaned out and sanitized before used.
The violations documented on this report will be checked during the next standard inspection.
A level 2 ODH certification in food safety training is required to be obtained for one operator of the facility by March 1, 2017.
