Comments
This inspection was completed per the the request of the owner to renew the MFSO license for the trailer.

MFSO was not in operation at the time of inspection. Inspection was completed with Kent Fire Prevention present. Hot water was observed at the 3 compartment sink and hand washing sink. Cold holding was observed to be at 39 degrees F.

The MFSO owner stated that all foods will be prepared on the MFSO and foods are purchased as needed per events. A processing kitchen/warehouse location is in the process of being finalized.

No violations were documented at the time of inspection.

Notes:
1) Ensure the potable water tank has been properly cleaned and sanitized before use. Observed a slight leak may be present. Water on the floor may be from filling to potable water tank.
2) Provide documentation regarding the warehouse/processor location that has been approved by ODA. Foods that are made or stored in a residential home are to be considered from an unapproved source and could be embargoed..
3) Current reach-in cooler is approved until replacement is needed. Equipment will need to be a NSF or equivalent testing agency approved unit when able.

A signed application and payment for the amount of $340 (Check#1190) was received at the time of inspection.
