3717-1-03.1(A)(3) / Sources - packaged food labeling
Critical Repeat Packaged food not properly labeled. Italian sub sandwich - Missing name and place or manufacturer, Allergen label is Dairy and not Milk. Turkey Bacon Avocado Sandwich Missing name and place of manufacturer. Kale Caesar salad missing name and place of manufacturer, Labeled to contain dairy and not milk. Farmer Market Salad missing name and place of manufacturer. Grape and cheese up missing name and place of manufacturer, labeled to contain dairy and not milk. Peanut butter and jelly sandwich missing name and place of manufacturer. Caprese Baguette missing name an place of manufacturer, labeled to contain dairy not milk. All label font is small and difficult to read. Font that is at least 1/16 of an inch in height based on the lower case letter “o.” Usually, this means a minimum of 6 pt font. Font cannot be three times as high as it is wide, or the words will be distorted and unreadable. Food was voluntarily destroyed by the manager at the time of inspection. Voluntary Destruction form was completed and is on file. All prepackaged that is made and transported other locations on campus shall not be sold until labels are correct.

3717-1-02.4(C)(16)/PIC: duties - ensure written procedures and plans are maintained and implemented in the facility.
Written procedures and plans are not maintained and implemented. Per the pre-licensing approval, the FSO was required to develop specific procedures. Lack of cold holding capacity requires daily deliveries from processing facility (KSU Catering Kitchen at Student Center/KSU Commissary Kitchen 2nd floor Tri-Towers). Processing Procedure on what will be delivered, how items will be transported, how often each item is to be delivered. All foods are required to date marked at the processing location. Procedure needs to address the containers used for holding TCS foods and how the staff is to respond to foods being delivered out of temperature or miss labeled.

3717-1-03.2(G)/Washing fruits and vegetables.
Raw fruit and/or vegetables not washed or washed improperly. PIC stated that produce is being washed/rinsed in the 3 compartment sink. The FSO has not been approved to us the 3 compartment sink to wash produce or be used as a food preparation sink. Until a procedure is approved, all foods must be washed and prepared at the processing kitchen.
3717-1-04.8(E)(3)/Single-service articles and single-use articles - storage.
Single-service and single-use articles not protected from contamination. Observed to go beverage containers were stored on the front counter removed from original packaging and exposed. Stored single use items in the original package or in a way that protect the lip contact area.
Comments
Follow-up inspection was delayed due to scheduling issues with REHS (illness).

Observed labels to not be correct. Observed a posted label that appears to cover the labeling. Foods are being manufactured at the same location with the same labels until the labels can be corrected. PIC stated that they will remove the foods from the labeled package until labels are corrected. This ensure that the customer is not be provided a incorrect label.

Ensure each location that is receiving foods from the "commissary location" has a written Processing Procedure for the staff to use when receiving foods. Ensuring proper labeling and temperature control needs to be included in the plan.

The Labeling violation and drain plugs will be re-checked at then next standard inspection. Other locations on campus with incorrect labels are at risk of embargo or voluntary destruction request. Correcting the label must be expedited.
