[?] A summary of the violations found during the inspection are listed below.

3717-1-03.4(F)(1) / Time/temperature controlled for safety food - hot and cold holding.
Critical Corrected During Inspection Observed TCS foods in the walk-in cooler (i.e. cooked peas, cooked chicken) were not being held at the proper temperature.
To prevent the growth of pathogens, except during preparation, cooking, or cooling, refrigerated TCS food shall be held at 41°F or less. The peas and chicken were discarded.

3717-1-03.4(E) / Cooling methods - temperature and time control.
Observed improper method for cooling TCS foods. Observed cooked beef was packed into a deep storage container in the walk-in cooler at the time of inspection.
To prevent the growth of pathogens: Cooling shall be accomplished by separating the beef into smaller or thinner portions and placing it in shallow pans. When placed in cooling or cold holding equipment, food containers in which food is cooling must be arranged so as to provide maximum heat transfer through the container walls and; loosely covered, or uncovered if protected from overhead contamination.
3717-1-04.1(H) / Nonfood-contact surfaces - cleanability
Repeat Observed nonfood-contact surfaces of shelving for equipment storage where contact paper is peeling was not designed and constructed to allow easy cleaning or maintenance at the time of inspection.
To protect against contamination, nonfood-contact surfaces shall be designed and constructed to allow easy cleaning and to facilitate maintenance. If shelf surfaces cannot be maintained easily cleanable, the equipment shall be replaced.
3717-1-04.4(A)(1) / Equipment - good repair and proper adjustment.
Observed equipment (walk-in cooler) was not maintained in good working order at the time of inspection.
To prevent foodborne illness, this equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code. Repair the walk-in cooler so that it maintains TCS foods at a temperature that is 41 degrees Fahrenheit or less. Correct By: 17-Aug-2015

Repeat Observed the floors of the walk-in cooler and the walk-in freezer were in poor repair at the time of inspection.
The physical facilities shall be maintained in good repair. Repair these floors so that they are maintained smooth, durable and easily cleanable.

Observed the ceiling above cleaned equipment and the drywall behind the warewashing sink were in good repair at the time of inspection. Consider installing FRP or similar smooth, durable, easily cleanable material in all areas subject to splash to prevent deterioration of surfaces.
